Use of 99354 in Primary care office

Hi,

I have a provider who saw an established patient face to face for 40 min. She did not have access to patient's medical records until time of visit. At the time of visit the patient gave her a giant stack of medical records to review with many complicated health conditions. I checked for NCCI edits on 99215 & 99354 and there are none. If the visit is based on time, is my understanding correct that patient would have to be seen face to face for 1 hour and 40 min to justify coding both codes? I read the description in the CPT book and am unclear.
