If I have a preventive visit e.g. 99397 and the visit also meets the criteria for an E&M e.g 99212 I know that I need to put modifier 25 on the E&M, but if I also have immunizations do I also put modifier 25 on the preventive visit?

No, you would not append modifier 25 to the preventive visit.
