Is anyone using the X modifier instead of the -59 for multiple polyp removal (45385, 45380)...how about when a polyp is injected and then removed by snare...45385, 45381...what modifier are you using in this situation?...is this paying?
 
x modifier

we are using the XU modifiers with all payors except for BCBS (we use the 59,XU) and we are being reimbursed
 
My colonoscopies w/different type biopsies on different parts of colon with the XS modifier is STILL denying... ugh
