I have a physician who performed a partial colectomy with low anterior anastomosis, small bowel resection, and excision of a peritoneal tumor. My question is, when a surgery session goes from laparoscopic to open, do you use Z53.31 on the primary procedure only? Or do you have to use it for every procedure performed? The session went from laparoscopic to open prior to anything being resected
 
If the provider was intending to do the entire procedure laparoscopic and needed to convert to open, I would use Z53.31 on all procedures as the last diagnosis.
