If a pt has surgery for a fracture, code 26605, and then casting on the same day, code 29075 - what modifier would I use?
 
Initial casting is bundled in the fracture care code.

from the casting and strapping guidelines in cpt manual

first cast/splint or strap application is included in the treatment of fracture and/or dislocation codes.

CPT codes for closed, percutaneous, or open treatment of fractures or dislocations include the application of casts, splints, or strapping. CPT codes for casting/splinting/strapping should not be reported separately.
