Hello,

Can anyone help me confirm the code ultrasound guidance to inject the trigonum? I have seen the code for the excision but I am looking to code the injection. I was thinking it was either 20606 or 20500 and 76942. Please help!

The Os Trigonum is a small bone (ossicle) that is seen in some people. It is located on the posterior aspect of the distal tibia at the ankle joint (Tibiotalar). In some people it can cause deep posterior ankle pain. Injecting it would be helped by Ultrasonic Guidance (76942) as it is deep and can be small, i.e. hard to get to. But the Ankle Joint is considered an Intermediate Joint as it applies to joint aspirations or injections, so 20605 would be the best.

Hope this helps.

Respectfully submitted, Alan Pechacek, M.D.
