Wiki using a modifier when intubation was done

I am from a hospitalist group and my physicians are constantly doing procedures(intubation, Central lines, thoracentesis, LP, paracentesis, etc)HOWEVER they also see the patient and address all of their other problems. I want us to get paid for the procedure and the visit. Which modifier would I use?

Significant, separately identifiable E/M

If they are called to the bedside to perform the procedure, then the E/M that goes with that procedure is bundled. If, however, they have performed and documented a significant, separately reportable E/M in addition to the procedure you can use a modifier on the E/M to identify it as such.
