Using a questionnaire as the hpi???

We have a new provider in our clinic and she is using a patient's questionnaire as the HPI. Can she do this? Although it is covering some of severity, duration and timing, it is still in a questionnaire format and not in the providers words or in paragraph form. I have never seen this done before. Is this compliant?
