It is my understanding that you can code the fracture as primary-post surgery with V code as secondary for follow-up and coding fracture as primary as long as the provider is following the patient for that reason. Any thoughts on this appreciated.
 
No that is incorrect. For ICD9CM you code only the V code once the fracture has been treated. So once the surgery is completed the follow up encounters are coded with the V code.
ICD-10 CM will be different. You will use the fracture code with the appropriate 7th character for the subsequent status.
