Would you use both 25 (since there is a 0 day global procedure - 15004) AND modifier 57 - 14040 on the E/M when the E/M is the same day as the procedure or would you use only modifier 57 on the E/M?

Yes, I'd use both (assuming that documentation supports both). Some and perhaps most payers will require both modifiers in order to allow the E/M service to unbundle from both the minor and major procedures performed that same day.
