I think I know this answer but CRS is hitting right now. Reviewing a hospital progress note, the hospitalist listed the pt's past medical history of CHF and COPD with no section for ROS or any other documentation under history that would qualify. I don't think "double dipping" applies to this if this documentation is used as past history under PFSH and the two systems, respiratory and cardiovascular, would be the ROS to give that part as detailed. Please either confirm I am right or I am off my rocker! :)
