I had a question if Dr. dx patient with a UTI and documented hx of UTI'S can I use both codes or only code for the UTI since its a confirmed dx?
 
Z87.440 would not be appropriate as the personal history code indicates a patient's past medical condition that they are no longer receiving any treatment for but has the potential for reoccurrence so needs to be monitored.

In this case the patient has an active UTI. You may want to look at chronic vs acute UTI depending on documentation.
