Can I code V58.69 (pt on meds for htn, anxiety long term) if we do not check lab for these conditions or treat these conditions? I work for a hand ortho spec clinic. Our rheumatologist will use the code and checks the pt labs, so I don't have any problem using for him. But, is it ok for the orthos to code this? They like to charge high level EM visits, and we document as much as we can for the pt history to justify the high EM visits.

the 58.6- codes are status indicators they only indicate a status of the patient being on those medications. If labs are checked and dosage changed then you also use the V58.83 for drug monitoring. So yes you may use the V58.6- codes to indicate that a patient is on certain medications.
