I am new to codding for optometry and have a claim for new eye glasses/lenses.

I am trying to figure out the correct modifiers to use for BCBS in GA.

Please advise if possible if he following is correct.

ICD-9 is: 367.1 - myopia

POS: CPT/HCPCS: Modifiers: Units:
12 V2020 51,NU 1
12 V2103 RT,NU,GA 1
12 V2103 LT,NU,GA 1

This is not a Medicare patient, so I'm not sure if I should use the GA modifier.
Also, I'm not sure is I need the modifier 51 on all lines or only certain ones.
Any advise is greatly appreciated :confused:
 
Call BC/BS of GA

Hi,
The best advise I can give you is to call BC/BS of GA and get their guidelines for submission of glasses. I do not know BC/BS of GA policies otherwise I would be able to give you a better answer.
