Is it appropriate to use V58.11 on the office visit when the visit is a pre-chemo visit with the doctor? When I took over billing for Hem Onc this was the practice for visits with the doctor to discuss chemo treatment or checking on the patient's status before the next treatment. Or is V58.11 only to be used for actual chemo infusion encounter? The code description includes (encounter or admission for chemotherapy).
