For Medicare we place V58.11 as priority 1 on the claim, does the same rule apply to Medicare products? Is this in writing somewhere?

V58.11 is a first listed only allowed dx code for all carriers. This is used for all visits for chemo administration. You may go to the dx coding guidelines to find this in writing.
