V67.2 Following chemotherapy

I am a little confused when to use this code. We have some payers stating they want the V67.2 on an office visit follow up to previous chemotherapy and the patient is presently receiving chemo. The heading for this codes says it includes survillance only following completed treatment. I dont understand what they mean by "completed", as in their entire treatment program or each individual completed treatment. The guidelines state for the follow-up codes, that they imply that the condition has been fully treated and no longer exists. Any thoughts? Can someone help clear this up for me? Thanks
 
Is the person between treatments convalescing? V66.2 or is treatments completed and now you're just following up? V67.2 that's the difference. Hope it helps
