Does anyone have a fee ticket used in your Peds office for the Docs to check the Administration code when giving a vaccine? I need to make one up, but thought surely someone has a ticket they could email me. Anyone? I am at ellen.roberts@louisville.edu

We don't have a seperate charge ticket for vaccines and administrations. They are all listed on our encounter form with all E/M codes and Dx codes common to our group.
