Hello, I'm looking for clarification on vaccines with an office visit. If the patient is new and the only diagnosis is Z23 for the vaccine is the e/m included in the administration code?

The chart has a complete history, Ros, and physical exam. It seems like significant and separate work to pull all of that information for a new patient.

Is it all included in 90460?

Typically speaking, the E/M code is bundled to the admin unless you have a modifier applied to the E/M. Also, watch your Dx codes because some payers won't pay if they share a Dx, even with modifier application.
