I am working on vaccines codes that have been denied by Medi-cal as being previously paid under a different code. However, there are no other vaccines billed at any time near the date of service. I am not sure why they are denying these vaccines codes.

Have you tried calling them, to ask? They can't tell you how to bill, but they can tell you what denied, and why. What codes did you bill, exactly? Some vaccines do bundle to one another, such as 90707 (MMR) and 90704 (measles only). Please provide more info, if you genuinely need help.
