I would still document it in the chart. When a child is adopted or has a name change the vaccine registry often deletes the patients record. We've had many cases where we are unable to locate a patient in the registry due to a name change. Also, patients have the option in my state to opt out of the registry and if they ever do that then you'll lose all records of your patients vaccine history.
