Is this a valid HPI? It is referring to a scar the doctor wants to check.

"The patient presents for follow up of history of skin cancer that was diagnosed. The surgical site is healed. Patient has a history of skin cancer located on the L sideburn cheek. Wants site checked"

I want to know if I can count this towards an office visit. Thanks

I would say it is brief, but it is valid HPI. Assuming normal findings the V code for personal history of skin cancer may explain the medical necessity for today's visit.
