We are doing an injection of Vancomycin, Ceftazidime, and Decadron, I'm wondering what J codes other offices use? The former billing person was using J3490 and listing them out on the claim sheet and it looks like they had a hard time getting this reimbursed. ?

Decadron injection, dexamethasone sodium phosphate, per 1 mg is J1100. Vancomycin per 500 mg is J3370 and Ceftazidime per 500mg is J0713.

What was the reason they were not billed separately?
 
Only Decadron is listed as IM, IV or OTH.

Yep, the intravitreal doses are much less than IV and there would be significant drug wastage. The doses are so minute the drug is drawn up using a tuberculin syringe that only holds 1 ml. 500 mg vials of Ceftazidime and Vanco are reconstituted and further diluted for administration, yet literature states Vanco has not been approved for intravitreal administration. Has anyone used J3590, unclassified biologicals?
