Does anyone have any insight w/ Vascular billing
with codes

35474 rt ( Rejected by Medicare saying inclusive)
35474 lt (Rejected by Medicare saying inclusive)
36247 Paid
75964 26 Paid
75962 26 Paid
76937 26 Paid

We are getting rejections from medicare should we be placing a 59 mod on these procedures?
