I have two questions maybe someone can answer them?? Thanks!!

Medicare B (CAHABA) is denying 37250 for primary procedure not identified on the claim? That is the primary code for IVUS the add on is 37251 any suggestions? (these are not being billed with coronary procedures but peripheral/vascular procedures)

Also Medicare B (CAHABA) is denying 75774 when it is billed multiple time, ex. 75774-26
75774-26-59

Any help would be great..

According to the 2013 & 2014 CPT book, both 37250 and 37251 are add on codes. Without knowing what the primary procedure is or what the report states, I'm not sure I could give sound advice on what is happening with 75774.

37250 is an add on code so you need to report it with a primary code. Read the paragraph above the code and you will see what I am meaning. on the 75774 what other codes have you reported? Maybe post the report with all the personal info deleted.
