If you are familiar with vascular coding, any help will be greatly appreciated.

Does CPT 36002 contain the reimbursement for Thrombin or it must be reported separately as CPT J3490?
Also, the same scenario with CPT codes 36470 and 36471. Do these codes already contain the reimbursement for the sclerosing solution?
I have noticed that Medicare denies them as bundled when managed care payors are covering them.
