When a new patient comes in for a vasectomy consult; what are most of you coding for these if they are scheduling for surgery? I was told my another coder to never give 99204s for vasectomy consults.

When billing a 99203 for vasectomy consult, what are you counting for the HPI, I can't seem to get higher than a brief HPI.

I would be interested to know what level is being used with the new guidelines. Is 99203 or 99204 most appropriate for a vasectomy sterilization consultation.
 
Same, 99203 here too. Desire for sterilization is the acute uncomplicated "problem", no data/labs usually, moderate risk for the procedure. Low MDM or level 3 is the highest in these cases.

With any service, you should level the visit based on the documentation for the individual patient at this specific encounter. Patient1 could be young and healthy with nothing else going on and could be a different level than Patient2 who is diabetic, hypertensive, and requires a discussion with their endocrinologist and further labs prior to scheduling a surgery. Level each encounter for problem, data and risk (or alternatively time) to come up with a final overall level of service.
Yes, most likely 99203, but 99204 is very possible.
