Wiki VATS Coding Question-CPT Code 32601 VS. 32653

Hi All - Looking for some guidance on a coding question I run into frequently. My thoracic surgeon performed an evacuation of a hemothorax. Most literature I can find recommends using CPT Code 32601. However, he states he removed a large amount of residual clot by suction and irrigation. Would this meet the criteria for CPT Code 32653 - removal of FB or fibrin deposit? I struggle with the use of one code versus the other. Any thoughts or experience on this would be greatly appreciated.

So he did it thoroscopic and no repairs were made?

That's correct - done thorascopically and no repairs were made. Just the removal of the residual clot. Thank you!

Then I would definitely go with 32653.
