Wiki Vein Therapy

Our interventional radiologist is going to be starting vein therapy and will be performing sclerotherapy, laser ablation and thermal ablation. My question would be does anyone know if medicare deems any of these procedures medically necessary? I have checked medicare policy and am confused as to if you have to meet certain requirements for each procedure and then if you do there are only four codes that will qualify as medically necessary?

We have been performing the laser ablation and the scerotherapy's for awhile and Medicare does pay if the patient does have medical necessity. We usually code these procedures with 454.8 and Medicare excepts that with no problem.

Could I use that ICD for revascularization? I have a arterial procedure and thats all the dr has used for his purpose.
