Does anyone know where I can find documentation that states we can only bill for 36415 once per date of service. My administrator is asking this question, if a patient comes in and we draw their blood, it is then discovered that we need to do it again, can we bill the 36415 twice with a modifier 59.

I don't think there's anything anywhere that says you can't, unless a private payor has a specific policy.

I would try using -76. I think that is a more appropriate modifier for this scenario.
