My office has been receiving denials on venipunture when billed with an E&M code. Should I be using a modifier 25 on the E&M, or a 59 on the venipunture and submit chart notes to support it?

What code are you using and which insurance? There are some insurances, no matter the code or modifer that will pay the Venipuncture.
