If a patient is going to receive the following can you tell me how to bill for it. I have read everything and i am thoroughly confused.
1- vial of wasp - will receive 6 doses
1 - vial of honey bee - will receive 6 doses
1 - vial of (yellow jacket, hornet, white hornet) - will receive 6 doses.
Do you bill 95147x18
or
95145 x 6
95145 x 6

A couple of medicare policies state "us a code below the venom treatment number only for "catch up" purposes.
I would be most grateful if I someone could help. Barbara
