When a physician reviews and verifies an encounter should the statement be specific or general from an auditing standpoint? Which phrase is more appropriate.

Specific example: I have reviewed and verified the patients chief complaint, review of systems, current and past medical, family, social, and surgical history at todays visit.

OR

General example: I have reviewed and verified the patients chief complaint and history at todays visit.
