Versus Diagnosis?

Hi All,
I have an issue of a particular Dermatology Provider (DTC Clinic) where she always use 'versus diagnosis' example below, please advise;

Assessment:
Polymorphous light eruption VERSUS Rosacea
Plan:
As patient states hc prn works, can keep with regimen. Add zinc oxide based sunscreen q80 minutes in sun.

'Versus' signifies that the provider has not determined which of the diagnoses has caused the condition for which the patient is seeking treatment. Since it indicates an uncertain diagnosis, neither of these can be coded (in a non-inpatient setting) and you would need to code the symptoms with which the patient presented.
