Can I accept 'waist circumference' rather than weight for one of the 3 required vitals on an E&M exam? One of my new psychiatrists is asking because it is not in a depressive client's best interest to have them get on a scale to weigh-in. Part of her argument is also that the waist Circumference, more so than the weight, is used to determine condition status in diabetics, which all makes sense. So, I'm leaning toward accepting this as one of the 3.

Wondering if anyone has read anything on the subject or what others think of this.

A couple of things to possibly consider.
1. The patient could stand on the scale backwards therefore not see the weight.
2. You can use either vitals or general appearance when scoring an E/M - so vitals are not required.
3. I don't think ALL vitals are required to use as a component, maybe only 2 or 3, like temp, bp, pulse...
4. Patients have the right to decline weight.
