Right MCA WADA testing: Initially Brevital IA infusion was carried
out with microcatheter position at the midportion of the right M1
segment. 3 mg of Brevital was slowly injected. Subsequently, language
and comprehension was tested. Subsequently after the patient's
strength was found to be normal at the contralateral hand an
additional 2 mg of Brevital was infused and memory testing was
carried out. The patient was clearly oriented to time and place and
was easily able to recite numbers 1 through 10 and easily able to
answer simple portions as well as recognize, objects. Memory testing
was also normal with the patient able to identify all 5 items and
words that she was previously asked to remember.

Left ICA Wada testing: Since the patient did not have either a
posterior communicating artery or fetal posterior cerebral artery
proximal internal carotid artery injection of the Brevital was
carried out. Intra-arterial infusion with 3 mg of Brevital produced
striking patient disorientation. The patient was unable to either
verbalize or comprehend any of the questions asked of her. She was
clearly unable to respond to her name or identify herself or any of
the objects she was shown. She remained aphasic and unable to
verbalize for

The WADA test itself is usually not done by the radiologists, but from what I understand, it (95958) is coded only once. The radiologist or other physician who does the catheterization prior to the WADA would code for the bilateral catheterizations and diagnostic angiogram (if performed).
