The insurance carriers have policies regarding the billing of these codes and at what interval age, you would want to check with this particular patients insurance carrier.

The majority of them are the same but don't quote me on that!
 
Well child

I would verify with the carrier as the carriers only allow one preventive visit per year. If the patient comes in for any reason other than well check then you would bill an e/m visit not preventive
 
I don't believe there is any specific 'coding' guidelines as to how often a well visit may be billed, however, as the others have stated each individual insurance carrier has an allowed number of visits each year. Most are one per calendar year or per policy year. You would have to check with each patients insurance carrier to determine how many may be billed and if they are even covered under their plan.
