Have you experienced limits on the number of well visits that insurance companies will pay? Where would a particular payor's policy on this be published? I've looked around and can't find anything to substantiate the assertion that the insurance companies limit these visits, and each insurance company/Medicaid plan is different.

Thanks, Cathy. I was hoping to be able to document some standards instead of everything being done one by one. It is much harder to systemically manage limits when you have to do everything one off.

Does anyone know if any of the payors, including Medicaid, have standard guidelines? What determines the number of visits?
