For billing a well woman exam for pap & breast exam. Patient had annual wellness earlier since it was too soon for pap & breast exam. Can an e/m code be used for commercial ins. since they can only have 1 annual wellness yearly. Any help greatly appreciated for commercial ins and also for Medicare.
