I am billing for a 99396 for this patient. Looks like she was also seen for cyst in a different office and told to see a GYN. The doctor reported a negative UA. Do I still need to charge for that or is that included in a well woman exam?

So what did he document with regard to the cyst? If he simply notes it and does not order tests to evaluate, then I am not sure you can come up with a separate significant E/M service in addition to the preventive visit. Let the documentation be your guide.
