I have a question for my fellow coders...I have a patient that came in and my doctor did a PAP on her; she coded it a well women visit but the pt has a vaginal discharge and ordor; can you still code a well visit 99395 with problems or should it be a reg office visit.

That would depend- did the provider also do additional work up that could qualify to bill an e/m and the well visit? If you review the CPT book for Preventive Medicine Visits 99381-99397 you'll see some verbiage that describes the instances/circumstances where an E/M could also be billed at the time of a preventive visit.

In this case, I would review the note/audit it to determine what should be billed.
