...when determining new or established patient? Scenario: first visit by a patient to practice was for flu shot only, billed only with the vaccine and admin codes, under Dr. A. using a group NPI. Patient comes back for sick visit in April. Dr. A is no longer with group, seen by Dr. B. Drs. A and B are of the same specialty. Dr. B coded as a new patient - I maintain that this is an established patient as pt has received services under group NPI. Who is correct?

You are correct. The patient is only 'new' to the new MD. He/she must honor the patient's status with the practice.
