Any ideas on the correct codes to bill for this procedure?
A minimally invasive, tenotomy was performed to treat pathologic degenerative plantar fascia tissue using the TenJet Hydrocision device.
The site was confirmed with the patient and marked with a marking pen. History and physical, as well as informed consent, were reviewed and placed in the chart. The point of maximal tenderness was identified and marked. The patient was transported to the procedure room, and a formal time-out was performed confirming
the RIGHT lower extremity as the correct operative site.
Using real-time ultrasound guidance, the Achilles tendon was identified along its course to its calcaneal insertion, demonstrating areas of degeneration.
The area was prepped in sterile fashion using chlorhexidine and anesthetized with 10 cc of 1% lidocaine with epinephrine using a 22g needle under ultrasound guidance.
An #11 blade was used to make an incision over the level of the affected tissue for access to the target tissue. A sterile sleeve was placed over the ultrasound transducer.
To treat the diseased tissue, the surgical instrument was introduced through the incision and advanced into the pathologic plantar fascia under continuous ultrasound guidance. Once appropriate positioning was confirmed within the diseased tissue,
a tenotomy of the plantar fascia was performed, with debridement and removal of degenerative fibers.
The device was moved back and forth and rotated under ultrasound guidance to treat all involved areas of tendinosis until satisfactory changes were observed on ultrasound imaging.
Ultrasound guidance was used throughout for localization and instrument guidance, and images were obtained and recorded.
Following the procedure, Dermabond was placed over the incision followed by a Tegaderm. Post procedure instructions were given to the patient who verbalized understanding. The patient tolerated the procedure well and was discharged from clinic in good condition.
