I was wondering what are the required elements of an annual in order to bill for a well woman visit? If the breast exam is not performed but the pelvic exam is can you still bill for a well woman?

Are you talking commercial or Medicare? Medicare has a list of 11 elements and you must do at least 7 to bill the G0101 code (Don't forget the Q0091 if you collect a pap).

As for commercial, it would be what is clinically/age appropriate. There are probably guidelines out there (ACOG would be a good place to start), but CPT does not provide any documentation guidelines.
