When a patient is seen for an annual exam and while in with the physician is seen for a SEPARATE diagnosis, what is the correct method of billing? What CPT code(s) would be used?

You would bill the Annual exam code and dx then the apporate e/m code with the dx and add MOD 25 to the E/M code. Hope this helps.
