We have scribes. Provider reviews the notes and signs. If the scribe then makes an addendum to the note (example- additional procedure documentation) and sends back to Provider to review, date and sign off on the addendum,
What is considered a reasonable time frame to complete this in order to be compliant? thanks in advance.
 
To my knowledge, there is no official CMS guidance that gives an exact timeframe. It states things like "timely" or "as soon as practicable."
Most health systems will have an internal compliance policy, and I would defer to that. If you are a private practice, I think setting a policy within 48 hours is reasonable. You could also make exceptions for situations where the provider is on vacation, etc.
In any situation, it should be signed BEFORE claim is submitted.
