Wiki What modifier do I add when billing 2 services for 2 providers in the same day

We offer multiple services in our office, If a patient comes in for an Office visit and physical therapy in the same day what modifier would I add so the payment for one does not get included for the payment for the other ? We would bill under the same facility but two different preforming physicians.

Add 25 modifier to the E/M visit.

I have added the 25, the claims still deny.

Is this for the facility claim or the providers' claim? If it's the facility claim, you shouldn't need a modifier on the E&M, but you'll need a GP modifier on the therapy charges if it's Medicare or a payer following Medicare guidelines.

Are they 2 different specialties? If they are, you can appeal with med rec's, if not only 1 visit will get paid
