My Dr. reported 33249 and 33244. The insurance company is denying 33244 for missing modifier. I am clueless! What modifier would be appropriate on this code?

I know some insurance company's automatically reduce the 2nd procedure but others will not. I would bill 33244-51. I add it regardless just to be safe when I am working any surgical procedures. Other than that I am not sure what other modifier they would want.
