What modifier should we include with CPT 28510?

Our provider is just billing CPT 28510 on a claim and it is getting denied by Horizon BCBS stating

The procedure code is inconsistent with the modifier used or a required modifier is missing. Usage: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.​

What modifier should we use for this CPT?

My guess is they are looking for the TA-T9 modifiers to indicate which phalanx/phalange was fractured:

TA Left foot, great toe
T1 Left foot, second digit
T2 Left foot, third digit
T3 Left foot, fourth digit
T4 Left foot, fifth digit
T5 Right foot, great toe
T6 Right foot, second digit
T7 Right foot, third digit
T8 Right foot, fourth digit
T9 Right foot, fifth digit
