Hi I am new to auditing.. here is my question .. if I receive a bill with minimal information on it .. as an auditor should try to refer or ask for the original documentation or should I try to contact the physician? Or give credit for what is there even if it is obviously under coded.

If you are an internal auditor, I would use the original medical record. If you are an outside auditor, then unfortunately you don't have that access and will have to just do your audit on what you have. But most definetly, if you have access to the original medical record, I would base my audit on that, but it is not necessary.
